A discharge coordinator is a professional who leads and implements the discharge planning process for patients within a health facility. The discharge coordinator provides education and support to the patient and hospital staff in the development and implementation of discharge plans. Discharge coordinators are part of the treatment team, and work with patients prior to discharge either from inpatient care or day hospital treatment. The discharge coordinator ensures that there are appropriate appointments for follow-up care, safety plans, and prescriptions for any medications that may be needed following discharge.
