Grand Rounds: Difficult to Treat Depression (DTD): Clinical Value and Research Challenges

Overview:

Many patients diagnosed with depression struggle to achieve lasting relief from their symptoms, despite various clinical treatments. It is common practice to refer to these cases to as "treatment resistant depression," though they might be better labeled as "difficult to treat depression" (DTD). The term DTD encourages a more thorough exploration of various obstacles—be they pharmacological, psychosocial, or biological/medical—that hinder sustained remission.This presentation will examine the DTD approach, which acknowledges that for some cases of depression, it might be more beneficial to focus on optimizing symptom management, enhancing quality of life, and improving daily functioning rather than repeatedly changing treatments that offer limited long-term benefits. We will also discuss clinical challenges, such as choosing suitable assessments for identifiable causes, and research challenges, including defining different types of DTD and evaluating their outcomes.
