Discharge and Continuing Treatment

Our treatment team works to establish a comprehensive care plan for each individual we serve. We strive to fulfill as many of each person’s needs as possible, from psychiatric and medical needs, to housing, to working with them to ensure they are receiving government entitlements, as appropriate. 

Upon discharge from the day hospital, we help patients find stable, secure housing within the community. We also work with each individual and his or her loved ones, to find effective and appropriate after care once they are ready for discharge.
