We begin planning for discharge as soon as possible so that we can ensure a smooth transition back into the community for your child.
We will likely recommend additional appointments with an outpatient provider for your child once they are discharged from our intensive program.
Our dedicated discharge coordinator will assist you in arranging initial appointments with outside providers. If an appointment falls during a time that is not suitable for you/your child, you are welcome to work directly with the outpatient provider to reschedule.
If you are not satisfied with the outpatient provider we have selected for your child, you can:
